Signal passing with no phasing almost certainly means the bias is incorrectly set. Make a mark on the trimpot so you can restore it if need be, then slowly turn it to both extremes. I bet the phasing comes back at some point in the trimpot travel.

Haha, it gets weirder now...

I hooked up a drum loop in order to have a constant signal and I was able to find the "sweet spot" with the trim pot, but the modulation is static and doesn't change by itself. I can however sort of mimic the phasing effect by turning the "Depth" knob by hand.

Okay, then the LFO is faulty. What could be the cause, Laurie? I would swap the old tantal capacitor as a first step, but is it in the LFO path? I cannot find the schematics for the PH-1 on the fly. Only for the PH-1R.

Agreed. Seems pretty certain the LFO isn't "O"ing.

This gets tricky - into the territory that needs a scope to really find out what is going on.

You could try replacing C11.

I'm assuming all the connections for all the wires to the pots are solid?

That dodgy solder joint (circled in yellow above) is on C12... if there is a problem with that joint, it could definitely affect the LFO.

Interesting! This pedal has had a rough life for very sure. The bottom plate was turned around for the velcro, I suppose? Not the worst method to save the optical state.
