That DA-2 was so dusty when I picked it up from under the display at my local shop. I have this little guitar shop around that's buying used stuff and puts it under display, old school style. Once I cleaned it, it looked MINT (I mean look at that glorious picture!).

My 2 cents is it was purchased from this closure and has been sitting under that display for 3-4 years now waiting for me to pick it up at a ridiculous price for its value. It was sold as used, but I'm pretty sure from the label, box, papers, timing and brief history of this underrated model this one is brand spanking new.

Tube screamers are labeled "Overdrive, Tone, Level". I'd have to go with one of the two you're mentionning.

Could it be an SD-1W? There was no mention of having a toggle switch or not...

This clue has been bugging me since the start. Haven't we seen this posted on the forum before? I know I've seen this somewhere but I am completely stumped.

My guess, either a Boss FA-1 clone or Morley wah.
